Voglibose is the oral antidiabetic drug used for the treatment type 2 diabetes mellitus .it is alpha - glucosidase inhibitor that is used to lower the sugar level in the blood in case of diabetic patients .Complex carbohydrates are converted into simple sugars (monosaccharide’s) which can be easily absorbed from the intestine. Therefore alpha glucosidase inhibitors reduces the impact of complex carbohydrates

Metformin belongs to the class of biguanids and used in the management of the type 2 diabetes mellitus. Metformin is the insulin sensitizer and reduces the insulin resistance. Metformin is beneficial in obese patients This combination together use to control the blood glucose level

Indications

diabetes mellitus

Mechanism of Action

Alpha- glucosidase inhibitors act as competitive inhibitors of enzymes which are present on the Border of small intestine needed to digest carbohydrates. The alpha-glucosidase undergoes Hydrolysis of oligosaccharides, trisaccharides, and disaccharides to produce glucose .the other alpha glucosidase inhibitor such as Acarbose, blocks pancreatic alpha-amylase and inhibiting alpha-glucosidases as well. Inhibition of these enzyme helps to reduces the rate of digestion of complex carbohydrates so that the lesser amount of glucose is absorbed Metformin prevents the hepatic glucose production, increases the intestinal absorption of the glucose and inhibit peripheral utilization of the glucose

Pharmacokinetics

Absorption – Voglibose is poorly absorbed from GIT but absorption of metformin is more by extended release
Metabolism – Voglibose is metabolized in lesser extent whereas metformin is also metabolized in the liver
Excretion- Metformin is excreted in unchanged form via kidney
