Each hard gelatin capsule contains:
Esomeprazole Magnesium 20 mg
Eq. to Esomeprazole
(As enteric coated pellets)
Domperidone
30 mg
(10 mg of immediate release pellets &
20 mg of Sustained release pellets)
Combination of esomeprazole and domperidone is used to treat gastroesophageal reflux disease in patients who are suffering from asthma

About Composition
Esomeprazole:
It is the proton pump inhibitor which prevent the hypersecretion of gastric acid from the stomach and reduces the chance of development of ulcers
Domperidone:
Domperidone is the specific dopamine antagonist. It is used as antiemetic. It increases the level of prolactin and fastens the peristalsis movement
Mechanism of action
Domperidone acts on dopaminergic receptor present on the chemo trigger zone, blocks its action and thus inhibit the occurrence of vomiting
Esomeprazole acts the suppression of acid by preventing the final step in gastric acid production by covalently binding to sulfhydryl groups of cysteines found on the (H+, K+)-ATPase enzyme at the secretory surface of gastric parietal cells. This effect leads to inhibition of both basal and stimulated gastric acid secretion, irrespective of the stimulus. As the binding of esomeprazole to the (H+, K+)-ATPase enzyme is irreversible and new enzyme needs to be expressed in order to resume acid secretion, esomeprazole's duration of anti- secretory effect that persists longer than 24 hours.

Absorption:
Esomeprazole and domperidone, both are absorbed by oral route.
Protein binding:
Esomeprazole is 97 % protein bound whereas domperidone is 91% bound
Metabolism:
Esomeprazole is metabolized in the liver with the help of cytochrome enzymes
Excretion:
esomeprazole is excreted by urine and via feces

Contraindications
- Domperidone is contraindicated in patients with cardiac disorders
- Contraindicated in pregnancy
- Contraindicated in people deficiency of vitamin b12
