Suspension Presentation

Each 5 ml contains:
Paracetamol IP                                   250 mg
Phenylephrine HCl IP                      5 mg
Chlorpheniramine Maleate IP      2 mg
Sodium Citrate IP                              60 mg
Menthol IP                                           1 mg


This Suspension contains analgesic-antipyretic Paracetamol with decongestant Phenylephrine and an antihistamine Chlorpheniramine maleate. Paracetamol produces analgesia by elevation in the threshold of pain and antipyretic effect by acting on the hypothalamic heat- regulating centre by inhibiting prostaglandin synthesis. Combination is used to relieve symptoms caused by the common cold, allergies, flu, or other breathing illnesses (such as sinusitis, bronchitis). Watery eyes, itchy eyes/nose/throat, runny nose, and sneezing can be treated using antihistamines.

Sodium Citrate is used as alkalinizing agent. It works by neutralizing excess acid in the blood and urine. It has been indicated for the treatment of metabolic acidosis. It also acts as a diuretic and increases the urinary excretion of calcium.

Menthol is cold receptor agonist and is used to treat sore throat, cold etc.

Mechanism of action

Paracetamol:

Paracetamol it inhibits COX-1 and COX-2 through metabolism by the peroxidase function of these isoenzymes. This results in the inhibition of phenoxyl radical synthesis which is essential for the cyclooxygenase activity of COX-1 and COX-2 and prostaglandin (PG) synthesis.

Phenylephrine HCl:

Phenylephrine is sympathomimetic drug and mimics the actions of epinephrine (commonly known as adrenaline) or norepinephrine. Phenylephrine selectively binds to alpha-1 receptors which leads to blood vessels to constrict leads to nasal decongestant action.

Chlorpheniramine Maleate:

Diphenhydramine acts on histamine H1 receptor as an inverse agonist. It structurally resembles the ethanolamine class of antihistaminergic agents. By antagonising the effects of histamine on the capillaries, it can reduce the tenseness of allergic symptoms. It also acts centrally as it crosses the blood–brain barrier and inversely agonizes the H 1 receptors which can result in drowsiness.

Menthol:

Menthol causes sensation of air passing through the nose, as it can make breathing easier during nasal congestion

Contrindications

  • Contraindicated in patients with known hypersensitivity
  • Contraindicated in diabetes mellitus
  • Contraindicated in glaucoma

Store it at room temperature protected from light
